Matching Gift Opportunity

This year, we are pleased to announce a matching grant opportunity with the Delta Dental of Kansas Foundation.

If you are a Delta Dental of Kansas network provider, the Delta Dental of Kansas Foundation will match up to $100 of your donation to the KDCF, up to a total of $25,000 for all KDA members. To encourage your participation in this opportunity, we have made it as simple as we can. Here is what you have to do:

As a KDA Member, when you receive your IRS thankyou letter, you will also receive an additional form. Complete Section I (Donor and Gift Information) of that form and include with it the bottom portion of the receipt letter and mail the form to:

All forms must be completed and received by Delta Dental of Kansas Foundation by December 31, 2008 to be eligible as a matching gift. We hope you will take advantage of this offer extended to us.
